TEMPLATE: Fall 2024 Academic Programs Use this template for programs/areas that have SLOs, PLOs, and FTES attributed to their program. *Note: even if your program/area is academic, if you don't have SLOs, PLOs and FTES, then use the Student Services/Administrative Services/Office of the President template.
TEMPLATE: Fall 2024 Student Services/Administrative Services/Office of the President Use this template for programs that have Service Area Outcomes. This template was updated on 9/10, so if you downloaded the template before 9/10, please re-download it to make sure you work on the latest version.
- Note: If your program/area has both SLOs/PLOs and SAOs, please reach out to the PAR tri-chairs and we'll help you determine which template to fill out.

Steps to Complete Your Program and Area Review
Step 1
- Download the Fall 2024 PAR electronic template for your area (e.g., Academic Services or Student Services/Administrative Services/OP) and save it to your computer. (Electronic PAR templates are located in the section above "Electronic PAR Templates.")
- Rename it with the name of your program or area.
Step 2
- Decide whether to use a collaborative platform.
- If you are working on your own, just save the PAR Template on your computer.
- If you are working with a team of people in your program/area or would like your manager/dean
to collaborate, then upload the template into a collaborative platform like OneDrive
or Google Docs and share access.
- Microsoft Suite OneDrive: How to upload and share a file on OneDrive.

Step 3
- Answer questions in the PAR template and any applicable forms, such as the:

- Faculty Prioritization Form
- Check out the data as you reflect on your program's/area's SLOs, PLOs, SAOs, and PAR goals.
- In this update year, you are not asked about any data point specifically, but reviewing
data on your program's: enrollment management, enrollments and success rates, degree and certificate awards, and more could help you assess how well you are doing with meeting SLOs, PLOs, SAOs, and/or
PAR goals.
- If you do not know how to use this data, then reach out to your area representative on the PAR Committee, your dean/manager, or the Office of Research, Planning, and Institutional Effeciveness (ORPIE).
- In this update year, you are not asked about any data point specifically, but reviewing
data on your program's: enrollment management, enrollments and success rates, degree and certificate awards, and more could help you assess how well you are doing with meeting SLOs, PLOs, SAOs, and/or
PAR goals.
- Service areas often collect their own data in Banner; be sure to use whatever data you have available to answer the questions in your PAR.
- If you will need any data from the ORPIE to assess your SLOs, PLOs, SAOs, or PAR goals, then request the data on the Research Request Form as early as you can (!), but by Friday September 20th at the latest. ORPIE will process requests in the order in which we receive them.
Step 4
- Recommend turning in your completed Electronic PAR Template to your manager/dean with the associated forms
early.
- Include resource requests, as noted in the word doc template.
- Include associated forms, such as the classified and faculty prioritization request forms.
- Wait for your Dean/manager to give you feedback. If you have not heard anything by Monday 10/14/24, you might want to reach out.
Step 5
- Submit your final PAR by 10/21/24, after your dean/manager has given you the go ahead, by going through the following steps:
- PAR Submission in Qualtrics: One person from your program's/area's PAR team should
submit the contents of the word doc template (except resource requests) in the Fall 2024 PAR Qualtrics Submissions.
- Plan to do this in one sitting; there is no "save" function in Qualtrics.
- Resource Requests in Cognito: Submit resource requests in the Fall 2024 Resource Request Submissions.
- Classified prioritization forms: upload with your resource request in Cognito forms.
- Faculty prioritization forms: email to Cheree Manicki cmanicki@chabotcollege.edu by the faculty prioritization deadline (9/30/2024).
- Discipline Plans: check with your dean to see if anything is required for discipline plans at this time.
- PAR Submission in Qualtrics: One person from your program's/area's PAR team should
submit the contents of the word doc template (except resource requests) in the Fall 2024 PAR Qualtrics Submissions.
Step 6
- Celebrate!!! You have completed your Fall 2024 Comprehensive Year PAR.
Fall 2024 Due Dates In Full
- 9/4/24 Program & Area Review Launch: Fall 2024 Templates Available on PAR Website
- Data Available via PAR or IR websites (with one exception, Degrees and Certificates data will be available in mid-September).
- 9/4/24 - 10/21/24 Area/Discipline PAR Teams Collaborate: to complete their PARs in the electronic template provided, seeking feedback from managers/Deans when needed.
- 9/7/24 - 10/21/24 Trainings: PAR Committee is available to provide trainings upon request.
- 10/21/24 Final PARs Due: After receiving approval from dean/manager, PAR team submits the information from the word doc template into Qualtrics and resource requests into Cognito.
- 11/5/24 Data Analysis Begins: PAR Committee analyzes data; begins developing synthesis statements
- 11/18/24 Dean Summaries and Division Resource Requests Due: Deans, please submit summaries on Qualtrics, and submit your own division resource requests in Cognito.
- 12/16/24 VP Summaries and VP Office Resource Requests Due: VPs, please submit summaries on Qualtrics, and submit your own office resource requests in Cognito.
- 12/20/24 Deans' Resource Request Assessment Due: Deans comment/assess applicable requests on the Resource Request Dashboard.
- 1/28/25 Synthesis Statements Target Date: PAR Submits synthesis statements to PRAC for their review.

College Planning Documents
- The following college planning documents may help you reflect on how your program fits with larger college goals and initiatives, as well as provide guidance for how you can frame funding requests in alignment with the college's goals.
Why did we start using Qualtrics for the Fall 2021 PAR?
- In brief: To promote excellent data analysis of college-wide trends
- In detail: Originally PARs were submitted on paper and the PAR committee and PRAC were left to analyze and make sense of reams of paper, then we moved to electronic templates and submissions, next, thanks to the hard work of those who have come before us (e.g., Patricia Shannon, Jan Novak, Wing Kam, and countless others), our own homegrown Chabot program review application was developed. This was a huge step forward for the college. As we continue to grow and get and review more and more data, we now need an app that allows for efficient data analysis of college-wide trends.
- Qualtrics allows for easy analysis and visualizations of the campus-wide data. By including more quantitative questions, and gathering the data on Qualtrics, the PAR committee (with the support of the Office of Research, Planning, and Institutional Effectiveness) is able to report back on campus-wide trends and achievements.
